Our mobile-friendly form takes approximately 5 minutes to complete. You'll be asked to provide basic information so we can match you with appropriate network providers:

• Legal name, date of birth, and Social Security Number (for identity verification by providers)
• Home address and contact information
• Employment or income information
• Active personal checking account details for direct deposit
• Requested care budget ($500–$5,000 — from third-party network providers based on eligibility)

Privacy Note: Your information is protected by 256-bit SSL encryption. We never sell your personal data to third parties for marketing purposes.

Review Network Offers

After submitting your request, you'll receive financing offers from third-party providers in our network. Each offer will clearly disclose:

• Your approved care financing amount
• The Annual Percentage Rate (APR) — ranging from 9.95% to 35.99% based on provider underwriting
• Monthly payment amount and due dates
• Total repayment amount over the life of the plan
• Applicable fee policies as determined by the provider

You are under NO obligation to accept any offer. Review everything carefully before connecting with a provider.

Connect With Your Provider

If you choose to accept an offer, you'll be connected directly with the third-party provider to complete their process. The provider's agreement will include full disclosures of all financing terms.

Receive Funds for Your Pet

Once the provider processes your signed agreement, funds are sent via ACH direct deposit to the checking account you provided. Most customers receive funds by the next business day. Exact timing depends on the provider and your bank's ACH processing schedule.

Repay Your Provider

Repayment periods offered by network providers range from 10 to 18 months. Payments are typically scheduled monthly and managed directly with your chosen provider. You can:

• View your remaining balance and payment schedule through the provider's portal
• Make additional payments or pay off your care financing plan early (confirm terms with your provider)
• Contact the provider's customer support team with any account questions

Prepayment: Most network providers offer no prepayment penalties. You typically only pay interest accrued to your payoff date—paying early generally saves you money.
